This happens mostly If you had pipboy mods (Pad Style) installed and didnt uninstall them correctly. Reinstall that mod and look at the moddescription how to uninstall them. it's possible a mod unequipped it from your character. Use player.additem 0011bacb 1 to add it to your inventory if it was removed.
